CMS MS 2024-2025 ATHLETIC PACKET is now DIGITAL AND INCLUDES THE FOLLOWING FORMS
- CMS Pre-Participation Examination Form (signed by student-athlete parent) and NCHSAA Pre Participation Examination form. (Must be Completed by Licensed Physician, Nurse Practitioner or Physician Assistant)
- Student -Parent Honor Code (sgned by student-athlete and parent/legal guardian)
- Completed green or blue insurance Notice & Release form acknowledging insurance coverage
- Student/Parent Concussion form (signed by both) Info sheet and SAPLG
- Copy of participation waiver form and Child Nutrition notification letter (if applicable)

REQUIRED FORMS FOR TRYOUTS
Before a student is allowed to participate in a tryout they must have all of the following forms completed on dragonflymax.com by the designated due date.  Once forms have been submitted for a sport. That paperwork will be good for all 2024-2025 sports seasons. Student-Athletes do not have to resubmit forms unless the physical has expired.
All forms must be FULLY COMPLETED on dragonfly to be eligible to tryout
Pre-Form Requirements:
- ALL student information
- ALL parent/guardian information
- INSURANCE IS REQUIRED!
- Seventh grade entry is REQUIRED! It does not matter if you are in 8th grade!!!!
- Initial every spot it says to initial!
- Sign every spot it says to sign!
- Make sure your physical has a date from the doctor (they do forget to date them. Not date=no participation)
- A sports physical and a school physical are two different things. CMS requires a SPORTS PHYSICAL from a doctor.
Blue Form (ALL SPORTS)
- The entire document must be filled out.
Green Form (Football)
- Entire document must be filled out.
Concussion Form
- Students must initial each box.
- Parents must initial each box.
- Student must sign and date.
- Parent must sign and date.
ALL PARTICIPANTSÂ MUST HAVE ADEQUATE PERSONAL INSURANCE OR STUDENT ACCIDENT INSURANCE IN ORDER TO PARTICIPATE.
ELIGIBILTY
6th Graders are not eligible to participate in any sports in any capacity whatsoever.
- 7th Graders are automatically eligible to participate their first semester of 7th grade. At the beginning of 2nd semester, they must meet all eligibility requirements.
- 8th Graders must meet all eligibility requirements in order to participate in tryouts.

TRYOUT PROCEDURES
- All tryouts are held for a minimum of 2 Days.
- Athletes must have a minimum of 2.0 GPA (C average) cannot fail 2 classes. I am looking at the last 9 weeks of school.
- No individual student is guaranteed any amount of days (i.e.; if a student shows up on day 2 of a tryout, they are allowed to participate, but if cuts are made that day they are eligible to be cut).
- Once a cut is made, no new students are allowed to tryout. No Exceptions.
- Students are strongly encouraged to bring any and all equipment necessary to enhance their chances to be selected for a team.
- Teams will post tryout information times, dates, procedures, and equipment requirements on their team page on this site.
- Rosters will be posted on the individual team’s page after final cuts are made on the same evening.
