DragonFly Registration Instructions

Athletic Dragonfly Paperwork

Includes the following forms that must be completed:

1. CMS Pre-Participation Examination Form
   • Signed by the student-athlete parent and NCHSAA Pre Participation Examination form.
   • Must be Completed by Licensed Physician, Nurse Practitioner or Physician Assistant
2. Student -Parent Honor Code
   • Signed by student-athlete and parent/legal guardian
3. Notice & Release Insurance Form
   • This form  is acknowledging insurance coverage
   • Typically blue or green in the printed packet
4. Student/Parent Concussion form and SAPLG
   • Signed by the Student-Athlete and the Parent
5. Copy of participation waiver form and Child Nutrition notification letter (if applicable)

All forms MUST be completed and submitted digitally to tryout. 

TIPS WHEN FILLING OUT FORMS:

1. Choose each sport that your child chose to participate in. 

• This will make it where you only fill out one form for the year. 

2. All paperwork, except the physical examination form, can be completed online.

• You must print out the physical exam form and give it to your doctor to complete.
• Once the physical is completed, you must hand-deliver, fax, or email a copy to the athletic director before registration is closed.
